Adult Family Care (AFC) pertains to a residential care program ideal for individuals deemed unsafe to live alone but do not need the support level provided in a group residential setting. Seniors seeking AFC have the option to reside with a host family or remain at home with their loved ones who provide daily care and support.

The care management team at Golden Days Social Circle will determine the eligibility of individuals seeking Adult Family Care by evaluating their physical, psychological, and medical needs. Host services may include assistance with Activities of Daily Living (ADLs), transportation, medication management, meal preparation, health monitoring, and many others.

To become eligible for AFC, clients must:

  • Be at least 18 years old
  • Have MassHealth Standard or MassHealth CommonHealth coverage
  • Have a diagnosis for a medical or psychological condition requiring daily supervision and support for ADLs, like:
  • Ambulating
  • Transferring
  • Bathing
  • Grooming / Dressing
  • Eating
  • Toileting

Please note that we require physician approval.

To become eligible for providing AFC services, host caregivers should:

  • Be at least 21 years old
  • Own or rent a home
  • Be single or married
  • Be employed elsewhere (optional)
  • Undergo a review process, including a home visit, background checks, and several visits with the individual requiring AFC
  • Agree to have a nurse or caseworker conduct monthly site visits
  • Complete orientation training, including CPR and First Aid Certification

Please note that the client’s spouse and those with guardianship status are not eligible to become host caregivers.

Golden Days Social Circle offers an Adult Foster Care program that provides top-notch support and solutions for clients, caregivers, and professionals. These include:

Financial Support – Since MassHealth funds our Adult Foster Care program, host caregivers can receive a monthly payment of $1,500 per client.

Health & Social Support – As your trusted AFC provider, we deploy a nurse and care manager who can render support for the clients, host caregiver, and/or their families. Our professionals will conduct monthly visits and provide on-call support 24/7. They can also answer health-related concerns and serve as a resource for medical education and training (when necessary).

Family Support – For clients who wish to remain residing at home, Golden Days Social Circle offers extensive support to help clients and their immediate families overcome the challenges of living with an illness or disability. We also strive to strengthen familial relationships in this service.

General Resource Support – Our nurses and care managers at Golden Days Social Circle can help determine the necessary resources for clients. These resources may include:

  • Mental Health Therapy
  • Transportation Services
  • Adult Day Care Services
  • Community Support Programs
  • Health Care Proxy Information (Guardianship)
  • Information About AFC Programs Beyond Our Service Area
